Financial policy:
All of our fees or co-pays are due and payable at the time treatment is rendered. Prior financial arrangements can be made for amounts over $200.
For our patients with dental insurance: We are happy to assist you in filing the necessary forms to help you receive the full benefits of your coverage; however, we can make no guarantee of any estimated coverage or payment. Because the insurance policy is an agreement between you and your insurance company, we ask that all patients be responsible directly for all charges. Please know that we will do everything possible to see that you receive the full benefits of your policy.

We are happy to offer a 5% prepayment courtesy for all treatment over $500 that is paid in full prior to treatment commencing.
Credit cards
We are happy to accept payment by MasterCard, Visa, American Express, or Discover. If you choose to prepay for treatment over $500 using your credit card, we can extend a 3% courtesy.
Other payment options
Prearranged monthly payments may also be arranged and billed to your credit card or checking account on a predetermined date each month.

Location:
We are located approximately half way in between LaGrange Road (Route 45 - East) and Wolf Road - West on the North side of 143rd Street. Coming from LaGrange Road, you will be turning right after the entrance to Crystal Tree subdivision onto Tamarack Lane, followed by an immediate left into our parking lot. We are located at the far western end of the lot. Coming from Wolf Road, you will be turning left after crossing 108th Avenue onto Tamarack Lane.

Our providers are not specialists in family, crowns, gum disease, sedation, prosthodontics, orthodontics,
endodontics, cosmetic, whitening, implants or non-specialty interest areas that require specific educational
